Diamond Quality Care works with NHS Hospitals and GP surgeries, Local Authorities, Private Funders, Solicitors, Local Communities, Insurance Advisors, Insurance Companies, Families, Occupational Therapists, Social Workers, Care Managers, Psychologists and of course our members.
There is a fully qualified Social Worker on site who is able to visit potential members on the hospital ward to assist with setting up a support plan prior to discharge as this often helps with the continuity of Care Support. In some circumstances, we are able to offer Care Management Services.
With Local Community waiting lists growing we have a fully equipped Gym and Aqua Therapy Pool and are able to give immediate support to our members who need to maintain a cardio, functional and mobility programme.
All Potential members are invited into the Centre for a discussion on their needs, goals and targets. This gives the member the opportunity to view and take an active part at the Centre where they are able to meet other members and our staff.
Diamond Quality Care is able to set out a schedule and value for money costing so that appropriate funding can be agreed prior to the new member starting their programme.
Programmes are always flexible as this takes into consideration the member’s health and well being on a daily basis.
Our Centre focuses on the enablement of our members who are able to work on their confidence, skills, and are able to adapt more confidently on their independence in a safe and friendly environment.
